Terms of Use

A health diary, not a diagnosis.

These terms set the ground rules for MED Diary, including subscriptions, AI estimates, and sharing with caregivers. They are written to be read, not merely accepted.

1. Agreement and Apple's license terms

By downloading, accessing, or using MED Diary, you agree to these Terms of Use. If you do not agree, do not use the app or server features.

MED Diary is distributed through Apple's App Store. These terms supplement the Apple Standard End User License Agreement (“Apple EULA”), which also applies to your license to the app. If these terms conflict with the Apple EULA on a matter governed by the App Store license, the Apple EULA controls. Apple is not responsible for operating MED Diary or providing MED Diary support.

2. Eligibility and authority

You must be at least 13 years old and legally able to agree to these terms. If local law requires a higher age, that higher age applies unless a parent or legal guardian validly agrees and supervises use.

If you record information for a family member or dependent, you confirm that you have permission and legal authority to do so. MED Diary does not determine whether you are authorized to hold or share another person's health information.

3. Health, treatment, and emergency limitations

MED Diary is not an emergency service. If you believe someone may be in danger or needs urgent medical care, contact local emergency services or a qualified healthcare professional immediately.

MED Diary is a personal recordkeeping and organizational tool. It is not a medical device, healthcare provider, clinical record system, diagnostic service, or substitute for professional judgment. Charts, ranges, trends, reminders, estimated A1C, BMI, insights, labels, and other app output are informational and may not apply to your circumstances.

Do not start, stop, delay, or change medication, testing, diet, or treatment because of MED Diary output. Confirm measurements, reference ranges, prescriptions, and treatment instructions with the original source and your qualified healthcare professional. You remain responsible for decisions made using the app.

Reminders, Bluetooth imports, Apple Health sync, notifications, and caregiver updates can be delayed, duplicated, incomplete, or unavailable. Do not rely on them where a missed dose, reading, or alert could create harm.

4. AI extraction and nutrition-estimate limitations

AI can misunderstand a report or photo, omit a result, choose the wrong unit, associate a range with the wrong test, or produce a plausible but incorrect answer. Lab extraction and meal nutrition results are drafts for your review, not verified clinical interpretations.

  • Compare every extracted lab value, unit, flag, date, and reference range with the original report before saving or acting on it.
  • Nutrition, carbohydrate, calorie, glycaemic, and portion estimates can vary substantially and should not be used for medication dosing or other high-risk decisions.
  • You decide what to redact from a lab report and must review the exact upload. Do not approve a report containing personal information you do not want processed by our service providers.
  • You must have the right to submit any report, description, or photo you provide.

5. Accounts, invitations, and Shared Access

Most MED Diary features need no MED Diary account. Shared Access uses Sign in with Apple so the service can bind an invitation to the intended people. Keep your Apple Account, devices, session, invite code, QR code, and encryption key secure. Tell us promptly if you believe they have been compromised.

The same Apple Account is one Shared Access identity across MED Diary and MED Diary Care. Deleting it from either app permanently removes every share that identity sent or accepted and the related encrypted relay records. It does not delete the private diary, iCloud data, or a MED Diary Premium subscription.

Shared Access is read-only and is not a professional monitoring service. A caregiver may not open the app, may see information late, and has no duty created by MED Diary to respond. The server cannot read end-to-end encrypted health-record content, but it can observe activity metadata described in the Privacy Policy.

You are responsible for choosing an intended recipient, entering the correct caregiver email, deciding which categories to share, obtaining any required consent, and revoking access when appropriate. Once a person sees, screenshots, exports, or otherwise receives information, revocation cannot make every copy disappear.

6. Free features, Premium, trials, and billing

Plans and prices

MED Diary is free to download and offers optional Premium features. Current reference pricing is US $4.99 per month or US $34.99 per year. The App Store controls the price, currency, tax treatment, and offer eligibility shown to you, so the amount in Apple's purchase sheet is authoritative and may differ by country or over time.

Without a subscription, MED Diary supports up to three current medications (active or paused) with daily or selected-day schedules, reminders, a daily checklist, and the Pharmacy Order calculator available from the Medication report/share sheet. The calculator uses medication schedules to calculate quantities for 7, 30, 60, 90, or a custom number of days. Free also supports up to three upcoming appointments with reminders. Plain-text Share/Copy from a real diary does not require Premium or consume a formatted-report allowance. Apple Health import and export, every MED Diary widget, viewing your latest vitals and logging new readings in the Apple Watch app, and every alternate app icon are also available without a subscription.

Premium removes the current-medication and upcoming-appointment caps; adds cycle and as-needed medication schedules and aggregate adherence insights; and covers optional MED Diary-created services and workflows such as live AI beyond the free allowances, direct integration with supported Bluetooth meters, cuffs, and scales, formatted report sharing and comparisons beyond their separate free allowances, creating or expanding Shared Access, and family profiles. Free users receive separate lifetime allowances of three formatted vital-report shares, three formatted medication-report shares, and three report comparisons.

Trial eligibility

Eligible customers who select the annual plan may receive a 7-day introductory free trial. Apple determines eligibility. If a trial is available, the app and Apple's confirmation sheet show its duration and the price charged afterward before you subscribe. Unless you cancel in time, the trial converts to the annual paid subscription.

Automatic renewal and cancellation

Payment is charged to your Apple Account at confirmation of purchase. Monthly and annual subscriptions automatically renew for the same period unless you cancel at least 24 hours before the end of the current subscription or trial period. Apple may charge the renewal to your Apple Account within 24 hours before that period ends.

You can view renewal details, change plans, or cancel in Apple's subscription settings. Cancellation takes effect at the end of the paid period unless Apple states otherwise. After Premium ends, existing diary records remain available, while new Premium actions may pause. An existing Shared Access share continues receiving updates until you narrow or revoke it; creating a new share, adding categories, or restarting a stopped share requires Premium. Uninstalling MED Diary, deleting diary data, or deleting a Shared Access account does not cancel the subscription.

Refunds, restoration, and price changes

Apple handles App Store billing and refund decisions. Use Report a Problem to request a refund, subject to Apple's terms and applicable consumer law. “Restore Purchases” can ask Apple to restore an eligible subscription on your devices. Apple will provide any notice or consent flow required for a subscription price increase.

7. Your data, exports, and privacy

You retain your rights in the health records, notes, reports, descriptions, photos, and other content you provide. You give MED Diary and its service providers a limited permission to process that content only to provide, secure, maintain, and support the features you request.

You are responsible for the accuracy, legality, and backup of your content and for protecting exported files. Deleting records can be permanent and may synchronize to other devices through iCloud. Apple Health keeps its own copy of data under your Apple settings.

Our Privacy Policy explains what data is processed, where it goes, retention, and your choices. The commitment not to sell health data or show advertising is also stated in our product philosophy.

8. Acceptable use

You may not misuse MED Diary or help anyone else do so. In particular, you may not:

  • access another person's data, account, share, invite, or device without permission;
  • use the service to violate privacy, intellectual-property, healthcare, export-control, or other law;
  • send malware, abusive traffic, automated scraping, or requests intended to bypass limits or disrupt the service;
  • probe, defeat, or interfere with authentication, encryption, rate limits, subscription checks, or security controls;
  • reverse engineer the app except to the limited extent the law expressly permits despite this restriction; or
  • represent MED Diary output as professional medical advice, a guaranteed result, or an official clinical record.

9. MED Diary ownership and feedback

MED Diary, including its software, visual design, written material, curated educational content, trademarks, and service technology, is owned by the developer or its licensors and is protected by applicable law. Your license is personal, limited, non-exclusive, non-transferable, and revocable under these terms and the Apple EULA.

If you send suggestions or feedback, you allow us to use them without restriction or payment, while we continue to handle any personal information under the Privacy Policy.

10. Availability, changes, and termination

We work to keep MED Diary reliable, but we do not promise uninterrupted availability, permanent compatibility with every device or accessory, or that every feature will remain unchanged. Services can be unavailable because of maintenance, networks, Apple or OpenAI services, hosting providers, device manufacturers, security events, or circumstances outside our control.

We may change, suspend, or discontinue a feature where reasonably necessary, including for safety, law, platform rules, security, cost, or technical feasibility. We will try to give appropriate notice when a material change affects paid service.

We may restrict server access if you materially breach these terms, threaten security or other people, or use the service unlawfully. Where reasonable, we will provide notice and an opportunity to correct the issue. Rights and obligations that by their nature should continue after termination will survive.

11. Disclaimers and limits of liability

To the fullest extent allowed by law, MED Diary and its server features are provided “as is” and “as available.” We disclaim implied warranties of merchantability, fitness for a particular purpose, accuracy, non-infringement, and uninterrupted operation. We do not warrant that health data, AI output, imports, reminders, sharing, or reports will be complete, timely, or error-free.

To the fullest extent allowed by law, the developer and its service providers will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages, lost data, lost profits, or personal injury arising from reliance on MED Diary where the law permits that exclusion. Any aggregate liability that cannot lawfully be excluded will not exceed the greater of the amount you paid for MED Diary during the 12 months before the claim or US $50.

Some jurisdictions do not permit particular warranty exclusions or liability limits. Nothing in these terms excludes rights, remedies, warranties, or liability that applicable law does not allow us to exclude, including mandatory consumer protections. The Apple EULA contains additional terms about warranty, product claims, and governing law.

12. Changes, questions, and support

We may update these terms as MED Diary changes. We will revise the date above and provide prominent notice of a material change where required. Continued use after the revised terms take effect means you accept them; if you do not accept them, stop using MED Diary and cancel any subscription separately through Apple.
